University Athletics Mission Statement

The mission of University High School Athletics is to provide student-athletes with an opportunity to compete in a high school athletic program that compliments their overall educational experience. More importantly, the UHS Athletic Department is strongly committed to the Six Tenets of Pursuing Victory with Honor: Caring, Citizenship, Fairness, Respect, Responsibility, and Trustworthiness.  Our athletic programs seek to instill good sportsmanship and citizenship, promote and develop leadership qualities on and off the field, and to foster discipline, dedication and hard work while striving to compete at the highest level. Here is our list of sports and coaches:

UNIVERSITY HIGH SCHOOL CHARTER 

ATHLETIC TEAM TRYOUT ELIGIBILITY PROCEDURES 

• STEP 1 - Per L.A.U.S.D policy regarding after-school athletic teams, students must complete and submit the 4 documents listed below, before the student is deemed eligible to tryout. No exceptions. 

• STEP 2 - All 4 Documents listed below must be submitted together, in one e-mail to your Head Coach before students can tryout.               

• STEP 3 - The Head Coach then submits the paperwork to the UNI Athletic Director and the L.A.U.S.D. Nursing Division for processing and final clearance. 

1. 1. ONLINE PAPERWORK – www.homecampus.com 

• Complete online athletic eligibility paperwork at: www.homecampus.com 
• Click on For Students & Parents icon at the top of the home page. 
• Click on the state of California icon on the left side of the next page. 
• Enter your username & password – or create an account if you’re brand new. 
• Drop down menus:     2023-2024 School Year

• Scan the Completion Verification Page verifying completion.
  

1. 2. PRE-PARTICIPATION PHYSICAL EVALUATION FORM 

• Must use the L.A.U.S.D. Pre-Participation Physical Evaluation Form. 
• Answer all 53+ questions, sign & date form at the bottom.  

• You must write an explanation to all the “Yes” answers to the questions on the lines provided on the bottom right side of the form. 

1. 3. PHYSICAL EXAMINATION PERFORMED BY A DOCTOR 

• Must use the L.A.U.S.D. Physical Examination Form (side 2, of 2-sided form) 
• Doctor’s Office fills out form, need doctor’s signature & office’s official ink stamp on Physical Form. 

1.  4. HEALTH INSURANCE CARD 

• Provide a scan of the student’s Health Insurance Card. (Kaiser, Blue Cross, Health Net, etc…) 
• Student Health Insurance is available for purchase through L.A.U.S.D approved vendor, if needed.
